Ion has checked in 4.2. I have incorporated David C.'s edits into 4.3.

I am taking the lock on 1-3 to incorporate people's edits, and will 
write a conclusion. I am passing the locks on 4.2 and 4.3 to David C. 
It looks like we have a bit of space, so there is space for David-fu 
there.
